What is the Primary Care Network?

A PATH TO BETTER HEALTHCARE.

The Primary Care Network (PCN) was established to advance the clinical and identified priorities of the ANHP OHT, in alignment with the Ontario Health Team (OHT) model and Ontario Health’s provincial expectations for Primary Care Networks.

The PCN is a collaborative advisory and action-oriented body that brings together interprofessional primary care providers and leaders from across the ANHP OHT. It serves as a mechanism to guide the implementation of a comprehensive, integrated, and culturally responsive primary care system that meets the needs of the Kenora region and surrounding First Nations communities.

The PCN will be community- and partner-based, rooted in trust, collaboration, and the shared vision of health transformation led by the ANHP OHT.

The role of the pcn

The PCN will serve as the ANHP OHT’s primary care advisory and implementation body. Its roles include:

  • Co-developing and guiding ANHP OHT’s implementation of the Primary Care Action Plan, aligned with Ontario Health’s expectations.
  • Advancing access to interprofessional primary care services, particularly for prioritized and underserved populations.
  • Supporting integrated care pathways across the continuum.
  • Enabling population health management and collaborative quality improvement (cQIP).
  • Building and maintaining strong partnerships across sectors (e.g., home and community care, acute care, mental health and addictions, Indigenous health).
  • Supporting data-informed decision making and sharing accountability for outcomes.
  • Utilizing population health management approach to identify enablers and barriers to health equity and access, with specific attention to the needs of Indigenous Peoples.
  • Advising on digital health adoption to support integrated care delivery.
  • Championing health human resource sustainability within primary care.

PCn Membership

Membership of the PCN consists of healthcare providers who are providing primary care. This includes:

  • Indigenous primary health care providers
  • Family Health Teams
  • Primary Care Physicians contracted under an ANHP Rural Generalist Council Agreement
  • Allied health professionals deemed to be part of the local PCN
