The purpose of the Data and Quality Improvement Working Group is to collaboratively review, monitor, and enhance safety and quality standards within ANHP. The working group engages ANHP organizations to cultivate a culture of quality, ensuring that the best available evidence is utilized to identify opportunities for improving care and patient experiences.
Through population health data collection, the group facilitates capacity building and partnerships to drive continuous, measurable quality improvement in patient outcomes.
Collaborative Quality Improvement Plan (cQIP)
The Data & QI group has developed a cQIP that incorporates population health management principles alongside local expertise to identify service gaps and enhance healthcare experiences and outcomes.
Provide supports to subcommittees or member organizations for implementation of QI initiatives, including the cancer screening group. This group was created as a direct subgroup of the Data and QI team, with clinicians and supports from primary care and mental health and addictions SPOs. The focus is to improve screening rates and to improve communication and collaboration between SPO’s as it relates to cancer screening, and reporting outcomes to local, regional, and provincial stakeholders.
Data Collection and Analysis
The Data & QI Working Group supports the ANHP team and partners in gathering and analyzing data necessary to fulfill funding agreement requirements and inform quality improvement efforts.
Implementation of Change Initiatives
The group also works to assist other ANHP working groups in executing change initiatives aimed at transforming the health system (Integrated Care Pathways, as an example).
The Data and QI working group has undergone a transition from a membership consisting of leaders from all ANHP partners, to a skill-based team comprised of clinicians, support staff and
management.
