Home & Community Care Working Group

The purpose of the All Nations Health Partners (ANHP) Home & Community Care Working Group is to collect information and recommend the development of a locally owned, integrated, sustainable and equitable Home & Community Care service delivery plan that improves the lives of the All Nations people within the geographic area of the ANHP.

Roles of the Home & Community Care Working Group

  • To work together as Health Partners to facilitate, support and design a collective vision for Home & Community Care, leveraging the 2020 work of Phase 1.
  • Provide transparent communication and advocacy for our stakeholders.
  • Ensure that the model demonstrates equitable access to Home & Community Care.
  • Increase cultural safety and eliminate systemic racism and discrimination, working in collaboration and a spirit of reconciliation.
  • Address social determinants of health that impact delivery of Home & Community Care.
  • Improve infrastructure, technology, and transportation.
  • Build strong relationships and partnerships across the continuum of care for all our communities, across provincial and federal jurisdictions.
  • Leverage and make recommendations for legislation and regulation.
  • Establish quality best practices.
  • Advocate for funding and leverage options.
  • Design and recommend a new model of delivery for Home & Community Care that will fit in with the ANHP integrated model of health in a community-based, people-centered framework.
  • Establish analytics and evaluation processes for any recommended delivery plans.
