What Is An Ontario Health Team?
Introduced by the Ontario provincial government in 2019, the Ontario Health Team (OHT) model streamlines and integrates services of healthcare providers in a defined geographic region.
The Ontario government is building a connected health care system centred around patients, families and care partners. OHTs, as part of that system, are intended to:
- strengthen local services
- make it easier for patients to navigate the system
- create seamless transitions between providers
Ontario Health Teams are groups of providers and organizations that are clinically and fiscally accountable for delivering a full and coordinated continuum of care to a defined geographic population.
Ontario Health Teams are being introduced to provide a new way of organizing and delivering care that is better connected to patients in their local communities. Working under Ontario Health Teams, health care providers (including hospitals, doctors and home and community care providers) work as one coordinated team – no matter where they provide care.
